Case Management/Service Coordination

Each individual's services are coordinated with the assistance of the Case Manager/QMRP. The Case Manager/QMRP monitors Individual Habilitation and Service Plans to ensure that each person is satisfied with the services they are receiving and feel that these services are bringing them closer to achieving their vision and what is meaningful in his or her life.

The Case Manager/QMRP advocates for each individual's rights and also helps the individual understand their rights so that they can advocate for themselves.

Health Care

Our Health Care Department offers LPN, RN and Certified Medication Aide services to all individuals served. Services are provided in a variety of settings including group homes, private homes and worksites. Assistance and training in all areas of self-medication are offered as well as assessments and medication administration. Private duty nursing may be provided with specific funding or on a private pay basis.
